LLNL Consultant Information
The Consultant Agreement format is specifically intended as a contractual agreement for independent/private consultants only. The individual's personal Social Security Number (SSN) will be used for tax purposes. An employer identification number (EIN) can ONLY be accepted for business entities registered with the IRS as a "sole proprietor."
The following documents are provided for your review:
- Travel Expense Rules (05/31/19) - LLNL Consultants must adhere to the guidelines presented in the Travel Expense Rules (with Travel Rate Schedule) whenever travel is required. Travel costs in excess of those allowed in the Travel Rate Schedule will not be reimbursed.
- Claim for Consulting Sevices and Travel Expense Report
- EFT - Electronic Funds Transfer - Direct deposit payments. Please complete and return to the Consultant Office.

Tax Forms
Obtain all required Federal and California Tax Forms directly from the IRS or FTB, or from the following Supply Chain Management General Provisions & Forms website which provides links to the IRS and FTB Forms and Publications websites: Tax Forms
Residency and Withholding Notes:
- Residency status will be determined by completing the Statement of Citizenship Form.
- 0% withheld if payments =/<$1,500; 7% on payments > $1,500 for consulting services performed in the State of California. No withholding for services performed outside of California. California Form 588 or 589 may be completed to request a waiver to all, or a portion of, any withholding.
